Application

Parents with children applying to St. Jude the Apostle School for the first time (including children who are siblings of current St. Jude students) are invited to complete an application. Applications must be completed online. Please note that children entering Pre-K must be four years old by September 30th.

Copies of the following items must be turned in with your application:

• Birth certificate
• Baptismal certificate
• Current immunization record, a written physician statement or a written dissent form
• If applicable, the most current academic grades (including standardized test scores).
• Additionally, a $160 application fee is required.
