Parents with children applying to SJS for the first time (including children who are siblings of current SJS students) are invited to complete an application form- available on our website beginning September 2020. Applications may be brought to the front office of St. Jude School, faxed to (225)769-0671, or emailed to

Copies of the following items must be turned in with your application: birth certificate, baptismal certificate, current immunization record, social security card, and, if applicable, the most current academic grades (including standardized test scores). Additionally, a $125 application fee is required.
